The information I have provided is true, complete, and accurate to the best of my knowledge.

•  I understand that providing false or misleading information may result in:

•  the withdrawal of government funding,

•  cancellation of my enrolment, and/or

•  being held liable for the full cost of the course.

•  I understand that my eligibility for funding under the Career Start Program is subject to the Queensland Government’s program rules and policies.

•  I am aware that under Career Start I may access up to two subsidised training places only, which may include:

•  one general qualification at Certificate III level and one apprenticeship/traineeship, or

•  two apprenticeships/traineeships (including school-based apprenticeships/traineeships), plus

•  one introductory-level skill set (if required).

•  I acknowledge that I cannot complete two general Certificate III qualifications under Career Start.

•  I consent to my information being shared with the Queensland Department of Trade, Employment and Training (DTET) for the purpose of confirming my eligibility, managing my training records, and reporting on program outcomes.

•  I understand that eligibility does not guarantee a training place, and that I may be responsible for paying student contribution fees as outlined by the training provider.

•  I understand that once I use my Career Start subsidised training entitlement, I cannot access additional subsidised places beyond the program’s limits.
